What doentation should I have been given regarding my son's 504 plan?

At this very time last year, I sent a letter to the school requesting a meeting to have my son with ADD evaluated for a 504 plan. We had the meeting...a psychologist from an outside source was there...some plans of action were made...Nothing that was discussed that day has been followed through with and my son will be attending summer school because 6wks before school was out the teachers informed me that he is failing 2 cles. I contacted the Department of Education to file a complaint and because I was never given any sort of doentation, I requested my son's records from the school. I was told that they do not have a 504 in place for him and that we have had a couple of care team meetings to discuss his complications...but nothing about pouting a 504 in place. I guess that explains why nothing was ever followed through with! What do I do now? After this meeting, I was not given doentation-but was also not told that my son did not qualify for help. The psychologist and teachers made a list of accommodations that would benefit my son...so I guess like an idiot I thought that is what would happen.
